On May 28, 2013 at approximately 1:15 p.m., a 2005 Dodge Magnum driven by JACK W. CATES JR, age 66, from Gearhart, was westbound on Highway 26 near milepost 36 when CATES reportedly fell asleep. The vehicle traveled across the eastbound lane narrowly missing an oncoming log truck, went over an embankment and rolled onto its driver side about 50 feet from the highway.

CATES was extricated by fire personnel and then taken by LifeFlight to Legacy Emanuel Medical Center for treatment of serious injuries. He was using safety restraints and the vehicle's airbags deployed.

OSP troopers from the North Plains work site are investigating. Senior Trooper Steve Carlson is the lead investigator.

OSP was assisted at the scene by Washington County Sheriff's Office, ODOT and fire personnel. One way alternating traffic was maintained for over 2 hours to keep the highway open during the emergency response and scene investigation.
